Home Health Care" means exactly what it states – medical care provided in a patient's home. HHC can include broad care given by skilled medical professionals, including skilled nursing care, physical therapy, occupational therapy and speech therapy. SMC & DC will extend services by arranging teams to visit homes to conduct assessment and investigations by a team of physician, nurse and physiotherapist and assist in setting up support medical equipment and ambulance transport assistance and other services for home healthcare.
